On or about May 22, 2018, at the above addressed jobsite, an employee using a pneumatic nail gun did not wear safety glasses, thereby exposing the employee to the hazards associated with eye injuries.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a.  On or about May 22, 2018, at the above addressed jobsite, employees performing residential roofing operations on a roof fifteen (15) feet and ten (10) feet five (5) inches above the ground were not protected from falls, thereby exposing employees to fall hazards.

On or about May 22, 2018, at the above addressed jobsite, the portable ladder used to access the roof did not extend at least three (3') feet above the upper landing surface, thereby exposing employees to the hazards associated with falls.

On or about May 22, 2018, at the above addressed jobsite, employees had been provided fall protection training, however, the employer failed to keep a written certification of record.
