29 CFR 1910.28(b)(3)(i): The employer did not ensure each employee is protected from falling through any hole (including skylights) that is 4 feet (1.2 m) or more above a lower level by one or more of the following: covers, guardrail systems, travel restraint systems, or personal fall arrest systems, as required by sub-items A through D of this section.    On or about May 30, 2018 and times prior thereto, employees performing sweep down at the lumber deck conveyor on the second floor of the saw mill were exposed to falls of greater than 4 feet at openings in the grating.

On or about May 31, 2018 and times prior thereto, employees required to handle lumber and wood were required to provide their own leather gloves. This condition exposed employees to contact hazards.

This condition exposed employees to caught in hazards.
