29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee use appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors or potentially injurious light radiation:    Worksite:  When an employee was using a cutting and grinding tool to cut stone slabs, the employer did not ensure that the worker wore adequate face protection.  During this work the tool "kicked back" and struck the worker above the eye, inflicting a laceration.  The worker wore only safety glasses, not a face shield which would have protected his entire face.

29 CFR 1926.300(b)(1): Power operated tool(s), designed to accommodate guards, were not equipped with such guards when in use:   Worksite: An employee using a hand-held cutting tool to cut stone slabs was injured when then tool kicked back and struck him in the face.  The tool was operated without its guard.
