29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladder(s) were not used at an angle such that the horizontal distance from the top support to the foot of the ladder was approximately one-quarter of the working length of the ladder:        a) On or about June 5, 2018, the horizontal distance (angle) from the top supports to the feet of a non-self-supporting ladder positioned on an 8/12 pitched roof during chimney work was greater than one-quarter of the working length of the ladder.        No abatement certification or documentation required for this violation.

29 CFR 1926.1053(b)(22): Employees using ladders were carrying objects or loads that could cause the employee to lose balance and fall:                a) On or about June 5, 2018, employees were exposed to the hazard of falling from elevations up to two stories (approximately 20 feet) when climbing a portable extension ladder from the ground to the roof while carrying another portable extension ladder (Keller 16 foot extension ladder model 3516-3) weighing approximately 30 pounds.     In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
