In the Maintenance Shop: On or about June 8, 2018, and at times prior thereto, employees were exposed to struck-by hazards, while utilizing slings to lift equipment/compressor parts were not taken out of service: a) In the Maintenance Shop: On or about June 8, 2018, and at times prior thereto, employees were exposed to struck-by hazards from utilizing sling which had evidence of excessive wear and frays, no markings regarding capacity and load bearing (sling blue in color Mfg. Tuff edge : 3 ft in length 1 inch wide), used to lift compressor parts. b) In the Maintenance Shop: On or about June 8, 2018, and at times prior thereto, employees were exposed to struck-by hazards from utilizing sling which had excessive wear and evidence of fraying and abrasions along the edges of the yellow sling, Mfg.Edgeguard :6 ft in length and 1 inch wide, used to lift compressor parts. c) In the Maintenace Shop: On or about June 8, 2018, and at times prior thereto, employees were exposed to struck-by hazards from utilizing sling which had evidence of worn stitching in load bearing area, sling blue in color Mfg. Tuff edge: 4 ft in length 2 inch wide, used to lift compressor parts. d) In the Maintenance Shop: On or about June 8, 2018, and at times prior thereto, employees were exposed to struck-by hazards from utilizing a round sling which had evidence of excessive wear and no manufacture markings regarding load capacity, size and manufacture.
