29 CFR 1910.95(k)(1): The employer did not train each employee who is exposed to noise at or above an 8-hour time-weighted average of 85 decibels in accordance with the requirements of 29 CFR 1910.95(k). The employer did not institute a training program and ensure employee participation in the program:  a) Erie Molded Plastics, Inc. - the employer did not provide hearing conservation training for permanent and temporary employees exposed to noise above the action level. On or about June 27, 2018, the following noise exposures were measured.  An operator was exposed to a mixed noise level of 87.2 dBA or an equivalent dose of 67.64%, as an 8-hour TWA, while operating LM2 and press 20. This level was based on 473 minutes of sampling; zero exposure was considered for the unsampled time; and    An operator was exposed to a mixed noise level of 87.1dBA or an equivalent dose of 66.50%, as an 8-hour TWA, while operating LM2 and press. This level was based on 469 minutes of sampling; zero exposure was considered for the unsampled time.

29 CFR 1910.95(l)(1): The employer did not make available to affected employees or their representatives copies of 29 CFR 1910.95 and did not post a copy in the workplace:  a) Erie Molded Plastics, Inc. - A copy of 29 CFR 1910.95 was not made available to employees exposed to noise above the action level. On or about June 27, 2018, the following noise exposures were measured.
