29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):  On or about June 14, 2018, the employer did not provide adequate safety training to employees that were required to operate the powered industrial trucks to prevent stuck by hazards.  Abatement certification is required for this item in accordance with the requirement of 29 CFR 1903.19(c).

29 CFR 1910.1200(e)(1): Employer had not developed or implemented a written hazard communication program included the requirements outlined in 29 CFR 1910.1200(e)(1)(i) and (e)(1)(ii):    On or about June 14, 2018, the employer required employees to work with chemicals such as, but not limited to, propane without developing and implementing a written hazard communication program. 29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  On or about June 14, 2018, the employer did not ensure employees were provided effective information and training on the hazardous chemicals such as, but not limited to, propane which can cause skin irritation and burns prior to being required to use.    No abatement certification is required for this item.
