29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:    On or about June 13, 2018, in the area Southwest of the building, where employees working from Haulotte Model HA46 RTJO aerial lift were exposed to a fall hazard while changing out the weather heads and conduits on a utility pole without a body belt with lanyard connected to an anchorage point on the boom or basket of the lift.
