29 CFR 1910.147(c)(1): Energy control program. The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.   In the production area, the employer did not develop a written lockout/tagout program describing how to lockout and control hazardous energy sources for affected and authorized employees.  Employees operated machines, such as but not limited to the Lantech Wrap machine, and a Rocheleau Blow molding machine, which exposed employees to the hazard of being caught in or struck by the equipment.

29 CFR 1910.178(l)(1)(i): The employer shall ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l).   At the establishment, in the shipping department, employees operating a Doosan Toyota 4000 pound forklift, serial number F8ADM-179000223, model number BO2UE, were not trained by the employer in the safe operation of the forklift, exposing employees to the hazard of being struck by the lift.

29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws:   At the establishment, in the production area, employees used a yellow extension cord supplying power to a Raver floor fan where the strain/tension pulled the outer protective sheathing away, exposing employees to electrical shock and fire hazards.

29 CFR 1910.1200(h)(1): Employers shall provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new chemical hazard the employees have not previously been trained about is introduced into their work area. Information and training may be designed to cover categories of hazards (e.g., flammability, carcinogenicity) or specific chemicals. Chemical-specific information must always be available through labels and safety data sheets. 29 CFR 1910.22(d)(1): On every building or other structure, or part thereof, used for mercantile, business, industrial or storage purposes, the loads approved by the building official were not marked on plates of approved design and securely affixed in a conspicuous place in each space to which they relate.  At the establishment, there was no load capacity sign attached to the 10ft. high overhead stroage area, exposing employees to the hazard of falling material.
