29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents: (a) Front Fascia: On or about 06/19/2018, and at times prior, the employer exposed employees to struck-by hazards while using pneumatic nail guns, in that protective eyewear was not provided to an employee operating this tool at 120 PSI while engaged in siding installation activities.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level shall be protected from falling to that lower level. Paragraphs (g)(1)(i) through (vii) of this section establish the types of fall protection to be provided to the employees on each type of scaffold. Paragraph (g)(2) of this section addresses fall protection for scaffold erectors and dismantlers. a) Front Fascia: On or about 06/19/2018, the employer exposed one employee to fall hazards of approximately 11 feet while installing siding material on a single family residential site without proper fall protection on top of a ladder jack scaffold.

The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards. a) Front Fascia: On or about 06/19/2018, the employer exposed one employee to fall hazards of approximately 11 feet while installing siding material on a single family residential site, in that proper fall protection training was not provided to affected employees.
