29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     a)105 Ball St: On or about June 19, 2018, an employee was exposed to falls up to 12 feet 9 inches eave to ground while replacing shingles on the roof of a residential home.  The employee was not protected from falling by a guardrail, safety net or personal fall arrest system.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    a) Back Porch: On or about June 19, 2018, employees were exposed to a fall of up to 7 feet 5 inches while using a 15 foot Werner extension ladder to access the roof to replace shingles.  The ladder extended approximately 1 1/2 to 2 feet above the roof edge.
