Dough hoppers did not have the entire opening protected with substantial grid-type guards to prevent the employee from getting his hands caught in moving parts, or the hopper shall be extended high enough so that the operator's hands cannot get into moving parts.    Location: 1105 53rd St North Bergen, NJ 07047-baking floor    a) Employee exposed to caught in hazards while operating on Piston Depositor machine AP-1044 ; absence of a machine guarding device resulted with an amputation on or about 6/20/18.
