29 CFR 1904.39(a)(2): Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, the employer did not report the in-patient hospitalization, amputation, or loss of an eye to OSHA.   On Saturday 7 April 2018, an employee was using a high pressure washer inside a tank when he jumped into the tank, triggering the nozzle, and cutting in half the big toe on his left foot. The employee was first taken to Caldwell Hospital by the Owner, and then by ambulance to St. Joseph Health Regional Hospital in Bryan, Texas. The employee required surgery to reattach the toe. The hospitalization and amputation was not reported within 24 hours to the local Area Office of the Occupational Safety and Health Administration. The Austin Area Office was notified on 8 June 2018 of the event, or approximately 2 months later.
