29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  On or about June 26, 2018, at a worksite located at 3245/3247 Groove Ave, Lorain, Ohio 44055, during residential roofing activities employees were exposed to fall hazards of approximately 18 feet. The employer failed to ensure fall protection systems/devices were used by all employees working on the roof.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:   On or about June 26, 2018, during the removal of roofing material on a residential site located at 3245/3247 Groove Ave, Lorain, Ohio 44055 the employer did not ensure employees working on a roof were protected from a fall of approximately 18 feet by providing fall protection training and/or fall hazard recognition.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.    On or about June 26, 2018, employees were exposed to a fall of approximately 12 feet. The employees were removing roofing materials on a residential roof located at 3245/3247 Groove Ave, Lorain, Ohio 44055. The employees were using a portable ladder to access the roof that did not extend three feet above the landing area.
