29 CFR 1910.178(l)(1)(ii): The employer did not ensure that each operator had successfully completed the training required by paragraph (l), except as permitted by paragraph (l)(5), prior to permitting an employee to operate a power industrial truck:(Construction Reference: 1926.602(d)) At the location of 1320 N. Tyler Road in Wichita KS, an employee removing a stone cutter that was placed on the scaffold with the use of a forklift (Skytrak 6042 telehandler #10355382) and employees on the scaffold were exposed to struck-by hazards. The employees were exposed in that the forklift operator did not complete forklift training.
