29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence:    (a) prepare for shut down by recognizing energy hazards and its controls,  (b) turn off and shut down the machines,  (c) physically locate and operate energy isolating devices in such a manner as to isolate the machine or equipment from the energy source,  (d) affix lockout or tagout devices to each energy isolating device,  (e) render safe all potentially hazardous stored or residual energy,  (f) verify that isolation and de-energization of the machine or equipment have been accomplished.    Production, screen printing machines: on or about June 27, 2018, employees assigned to add ink (service and maintenance) to color trays were exposed to caught-in and struck-by hazards while hazardous energy controls (lockout) were not done in the sequence as required by 29 CFR 1910.147(d)(1)-(6).    Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.132(e): Defective and damaged equipment. Defective or damaged personal protective equipment shall not be used.  Production, spray blast/cleaning work station: on or about June 27, 2018, employees spot spraying ink soiled apparel with a cleaning fluid were provided with a single pair of damaged (cut) gloves for shared use. Employees' fingers and hands were exposed to contact with the cleaning fluid likely to result in skin irritation.
