29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) On or about June 28, 2018: Three employees were observed installing floor joist trusses and were not protected by guardrail systems, safety net system, or personal fall arrest system, nor was employee provided with an alternative fall protection measure, the employees were exposed to a 9-feet fall hazard.   b) On or about June 28, 2018: Two employees were observed installing floor joist trusses and were not protected by guardrail systems, safety net system, or personal fall arrest system, nor was employee provided with an alternative fall protection measure, the employees were exposed to a 20-feet fall hazard.  Big Hawk MX Builders LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1276494, citation number 1, item number 1 and was affirmed as a final order on February 9, 2018, with respect to a workplace located at 7869 Timberview Loop Wesley Chapel, FL 33543.  Big Hawk MX Builders LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1293000, citation number 1, item number 1 and was affirmed as a final order on July 18, 2018, with respect to a workplace located at 18573 U.S. Hwy 19 Clear Water, FL 33764.

29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.  a) On or about June 26, 2018: Five employees were installing floor joist trusses at heights above six feet from next lower level which exposed them to a fall hazard and the employer did not provided documentation on the training provided to such employees that will allow them to recognize such hazards and equipped them on how to minimize them.  Big Hawk MX Builders LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1276494, citation number 2, item number 1 and was affirmed as a final order on February 9, 2018, with respect to a workplace located at 7869 Timberview Loop Wesley Chapel, FL 33543.
