29 CFR 1904.29(b)(3): The employer did not enter a recordable injury/illness on the OSHA 300 Log and complete an OSHA 301 Incident Report or equivalent within seven (7) calendar days of receiving information that a recordable injury or illness has occurred:    For a carbon monoxide exposure illness that was reported on June 30, 2018, an entry was not made on the OSHA 300 log as of July 9, 2018 when the log was provided to OSHA.    Abatement documentation is required for this violation.

29 CFR 1910.1200(h)(2)(ii): The employer did not provide information to the employees on operations in their work area where hazardous chemicals were present:  In the production room, where a gas fired oven was used to cook product and carbon monoxide was measured up to 44 ppm on July 6, 2018, information on the presence of carbon monoxide, source of the gas, levels of gas including exposure limit, health hazards of carbon monoxide (including review of a safety data sheet), and the effective methods to control and minimize gas levels was not provided to employees.  Abatement documentation is required for this violation.
