29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    On or about July 10, 2018, employees were exposed to a struck by hazard while installing vinyl siding materials on a new residential home located at 4595 Lakeside, Cuyahoga Falls, Ohio 44221. Employees were working from a pump jack scaffold using tools and materials that could have fallen to lower employees and the employer did not ensure that the lower employees were wearing  protective helmets.

29 CFR 1926.452(j)(2): Poles were not secured to the structure by rigid triangular bracing or equivalent at the bottom, top, and other points as necessary:    On or about July 10, 2018, employees were exposed to a fall of approximately 14 feet while installing vinyl siding materials on a new residential home located at 4595 Lakeside, Cuyahoga Falls, Ohio 44221.  The employees were working from a pump jack scaffold and the employer did not ensure that the poles of the scaffold were secured by rigid triangular bracing or equivalent, at the bottom.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:    On or about July 10, 2018, employees were exposed to a fall of approximately 14 feet while installing vinyl siding materials on a new residential home located at 4595 Lakeside, Cuyahoga Falls, Ohio 44221.  The employees were working from a pump jack scaffold and the employer did not ensure that they were using fall protection.     Chris Changet Siding was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(g)(1)(vii), which was contained in OSHA inspection number 1316681, citation number 1, item number 1 and was affirmed as a final order on 6/5/2018, with respect to a workplace located in North Royalton, Ohio 44133.    Chris Changet Siding was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(g)(1)(vii), which was contained in OSHA inspection number 1181238, citation number 1, item number 1 and was affirmed as a final order on 11/1/2016, with respect to a workplace located in Seven Hills, Ohio 44131.
