29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds shall be fully planked or decked between the front uprights and the guardrail supports as follows: On or about July 10, 2018 at 1511 Royal Palm Drive Slidell, LA 70458; the employer did not ensure that each supported scaffold working level was fully planked between the platform and the uprights. Employees were accessing scaffolding to apply stucco to side of a multi story residential home.

29 CFR 1926.451(e)(1): When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface shall be used. Cross braces shall not be used as a means of access. On or about July 10, 2018 at 1511 Royal Palm Drive Slidell, LA 70458; the employer allowed employees to work from a scaffold that did not have a ladder as a safe means of access or egress. Multiple levels of scaffolding were used to access the multi level residential home for the application of stucco.

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees other than erection/dismantling crews: On or about July 10, 2018 at 1511 Royal Palm Drive Slidell, LA 70458; the employer allowed employees to access the second level of scaffolding to access a multi-level home, while not wearing fall protection or protected by a complete guard rail system on each side of scaffold. Employees were using multi level scaffolding to access the exterior of a two story house.
