29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:   a) On or about 07/13/18, at a 2-story house located at 77 East End Ave., Buffalo, NY: An employee was standing on the aluminum pic of a ladder jack scaffold erected on the north side of the house while installing roofing shingles near the lower edge of the roof. The employee was wearing a full body harness, but was exposed to a fall of 12 ft. 4 inches to the ground because he was not tethered to an anchor point.  No other fall protection system was utilized.     NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.452(k)(5): Scaffold platforms were bridged on to another:   a) On or about 07/13/18, at a 2-story house located at 77 East End Ave., Buffalo, NY: An employee was standing on the aluminum pic of a ladder jack scaffold erected on the north side of the house while installing roofing shingles near the lower edge of the roof.  The employee was exposed to a potential fall or tripping hazard on the scaffold because the aluminum pics were overlapped (bridged).     NO ABATEMENT CERTIFICATION REQUIRED
