29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  On or about July 20, 2018, Landers Construction and Remodeling, Inc. had an employee exposed to fall hazards in excess of 10 feet without fall protection, on a commercial construction site in Springfield, Illinois.

29 CFR 1926.502(d)(17): The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head:  On or about July 20, 2018, Landers Construction and Remodeling, Inc. had an employee exposed fall hazards in excess of 10 feet, and the employees attachment point of the body harness was at the waist and not in the center of the employees back or above the employees head, on a commercial construction site in Springfield, Illinois.
