The condition was most recently observed on or about July 18, 2018, at J.P. Bushnell Packing Supply Company, 3041 Locust Street, Saint Louis, Missouri.  The employer does not ensure employee(s) connecting a 30-amp, 4-prong, wire to a Frigidaire, 27-inch Electric Laundry Center Dryer (Model No.: FFLE3900UW, Serial No.:  4E82111556) during an appliance delivery and installation, do so in accordance with instructions included in the listing and labeling.

The employer is failing to prevent employee exposure to contact with electricity. The condition was most recently observed on or about July 18, 2018, at J.P. Bushnell Packing Supply Company, 3041 Locust Street, Saint Louis, Missouri.  The employer does not train and ensure employee(s) are familiar with the safety-related work practices required for connecting a 30-amp, 4-prong, wire to a Frigidaire, 27-inch Electric Laundry Center Dryer (Model No.: FFLE3900UW, Serial No.:  4E82111556) during an appliance  delivery and installation.

29 CFR 1904.39(a)(1): The employer did not report within 8-hours the death of an employee resulting from a work-related incident:        The employer is failing to report a work-related fatality within 8 hours.  Most recently, on or about July 18, 2018 at J.P. Bushnell Packing Supply Company, 3041 Locust Street, Saint Louis, Missouri an employee was injured in a work-related incident and pronounced deceased at the hospital.  Notification of the fatality was made by the next of kin and not the employer.
