The employer did not annually certify that operating procedures were current and accurate.     On or about July 26, 2018, at Big Lake Gas Plant, LP located in Big Lake, Texas, the employer did not annually certify operating procedures to ensure that they are current and accurate.  The operating procedures have not been certified since 2000 for operations that included, but not limited to the following:    a.  Initial startup  b.  Emergency shutdown,  c.  Normal shutdown,  d.  Loading and unloading  e.  Startup procedures following a turnaround or after emergency shutdown.
