29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:      A.)  Formtech Enterprises, Inc.; Line 1 - On or about 8/1/18, and at times prior thereto, the puller (P8) did not have a guard over the ingoing nip points.    B.)  Formtech Enterprises, Inc.; Line 8 - On or about 8/1/18, and at times prior thereto, the Rockwell band saw did not have an adjustable guard to cover all sections of the unused portion of the saw blade and blade guide wheels.

29 CFR 1910.334(a)(2)(ii): There was a defect or evidence of damage that could have exposes an employee to injury and the defective or damaged item was not removed from service and an employee was using it before repairs and tests necessary to render the equipment safe were made:      A.)  Formtech Enterprises, Inc.; Line 1 - On or about 8/1/18, and at times prior thereto, an electrical cord was being used to power lighting on a work table.  The cords outer jacket had pulled away from the plug.
