The employer failed to establish and implement a written program and procedures for lockout and tags-plus systems to control hazardous energy during the servicing of any machinery, equipment, or system in shipyard employment: a) Jobsite: The employer failed to develop and document energy control procedures during trouble shooting activities to prevent an employee from being exposed to gravitational energy when performing vessel T-girder research and development. Condition was noted on or about 6 AUG 2018.

29 CFR 1915.89(c)(1): The employer did not ensure that, before any authorized employee performed servicing when energization, or the release of hazardous energy could occur, that all energy sources were identified and isolated, and the equipment was rendered inoperative: 1) Jobsite: Energy isolating devices used to control gravitational energy, and the potential conversion of gravitational energy to kinetic energy were not used. A steel T-girder that weighed approximately 1300 pounds was not restrained/secured during trouble shooting activities to prevent displacement; thus exposing an employee to crushing hazards. Condition was observed on or about 6 AUG 2018.
