29 CFR 1904.32(a)(1): At the end of each calendar year the employer did not review its OSHA 300 log to verify that the entries were complete and accurate:    (a)   Choice Electric Corporation at 2080 W. 60th Ave., Denver, CO 80221:  On and before August 6, 2018, the employer did not review its OSHA logs for, but not limited to, years 2015 and 2017 to ensure the  corresponding Summaries of Work-Related Injuries and Illnesses (OSHA Form 300-A) were accurate and complete in that both Summaries were missing data.

29 CFR 1904.41(a)(2): The establishment had 20 or more but fewer than 250 employees at any time during the previous calendar year, and the establishment was classified as an industry listed in appendix A to subpart E of this part, but did not electronically submit information from OSHA 300A Summary of Work-Related Injuries and Illnesses to OSHA or OSHA's designee.  The employer did not submit the information once a year, no later than the date listed in paragraph (c) of this section of the year after the calendar year covered by the form:  (a) Choice Electric Corporation at 2080 W. 60th Ave., Denver, CO 80221:  On or before August 6, 2018, the employer  failed to electronically submit information from their OSHA Form 300A Summary of Work-Related Injuries and Illnesses or equivalent for year 2017 on or before July 1, 2018.
