29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:    Employees were exposed to respiratory illness due being required to wear N-95 type respirators.   The employer did not develop a written respiratory protection program that included the following provisions:      1) Procedures for the selecting respirators,  2) Medical Evaluation for employees using the respirators,   3) Fit Testing Procedures   4) Procedures and schedules for maintaining respirators,   5) Procedures to ensure adequate air quality,   6) Training of employees on respiratory hazards,   7) Training of employees in the proper use of the respirators.
