29 CFR 1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  On or about August 13, 2018, employees were exposed to a struck by hazard while installing roofing materials on a residential home located on S/L 5 Chesterfield, Chippewa Lake, Ohio 44215. The employees were using air powered nail guns and the employer did not ensure that they were wearing protective safety glasses.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).  On or about August 13, 2018, employees were exposed to a fall of approximately 19 feet while installing roofing materials on a residential home located on S/L 5 Chesterfield, Chippewa Lake, Ohio 44215. The employees were working on all areas of the roof and the employer did not ensure that they were using fall protection.
