29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.  On or about April 8, 2019, employees were exposed to a fall of approximately 12 feet while installing roofing materials on a new residential home located on SL 59 Crimson Harvest, Medina, Ohio 44256.  The employees were using an extension ladder to access the roof and the employer did not ensure that it extended 3 feet above the landing area.

29 CFR 1926.102(a)(2): The employer did not ensure that each affected employee used eye protection that provided side protection when there is a hazard from flying objects.  On or about April 8, 2019, employees were exposed to a struck by hazard while installing roofing materials on a new residential home located on SL 59 Crimson Harvest, Medina, Ohio 44256.  The employees were using air powered nail guns and the employer did not ensure they were wearing safety glassed.   ALM Roofing and Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard CFR 1926.102(a)(1), which was contained in OSHA inspection number 1338868 citation number 1,  item number 2  and was affirmed as a final order on 10/2/2018, with respect to a workplace located at SL 5 Chesterfield, Chippewa Lake, Ohio 44215.      ALM Roofing and Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard CFR 1926.102(a)(1), which was contained in OSHA inspection number 1338874 citation number 1, item number 2 and was affirmed as a final order on 10/9/2018, with respect to a workplace located at 6796 Maple brook, Medina, Ohio 44256.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).  On or about April 8, 2019, employees were exposed to a fall of approximately 12 feet while installing roofing materials on a new residential home located on SL 59 Crimson Harvest, Medina, Ohio 44256.  The employees were working on all areas of the roof and the employer did not ensure that they were using fall protection.
