29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, were not protected by protective helmets. On or about August 21, 2018, employees working on a single story commercial building were exposed to head injury while working in areas where overhead work (5 feet 8 inches above) was in progress such as but not limited to employees working on scaffolding.

29 CFR 1926.451(b)(2): Except as provided in paragraphs (b)(2)(i) and (b)(2)(ii) of this section, each scaffold platform and walkway shall be at least 18 inches (46 cm) wide. On or about August 21, 2018, employees working on a single story commercial building were exposed to falls of approximately 5 feet 8 inches to the ground below when working on a job made scaffold platform that was not at least 18 inches wide.
