29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    On or about August 23, 2018, to access the working level, employees were utilizing a portable fiberglass extension ladder that was not extended three feet above the landing surface.

29 CFR 1926.502(k)(5): The fall protection plan did not document the reasons why the use of conventional fall protection systems (guardrail systems, personal fall arrest systems, or safety nets systems) are infeasible or why their use would create a greater hazard.  On or about August 23, 2018, the employers fall protection plan did not document infeasibility when performing tasks such as but not limited to: removal of fall protection from a rooftop level.
