29 CFR 1926.102(a)(2): The employer shall ensure that each affected employee uses eye protection that provides side protection when there is a hazard from flying objects. Detachable side protectors (e.g. clip-on or slide-on side shields) meeting the pertinent requirements of this section are acceptable.  Residential construction site: On or about August 23, 2018, and at times prior thereto, employees were exposed to struck by hazards from flying objects while installing roof tiles without safety glasses.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   Residential construction site: On or about August 23, 2018, and at times prior thereto, employees without fall protection were exposed to a fall hazard greater than 6 feet to the unpaved ground below while installing roof tiles.
