On or about 8/24/2018 at 1470 Kaden Lane, Lot 77, Braselton, GA where the employer failed to ensure that workers observed performing framing operations at 19-feet above the lower inside level and 30-feet to the lower outside level were connected on the 6/12 pitch roof.    b. On or about 8/24/2018 at 1460 Kaden Lane, Lot 78, Braselton, GA where the employer failed to ensure that workers observed performing framing operations at 19-feet above the lower concrete level were connected.    c. On or about 8/24/2018 at 1460 Kaden Lane, Lot 78, Braselton, GA where the employer failed to ensure that workers observed performing framing operations at 19-feet above the lower concrete level wore fall protection.

The employer failed to provide ladder training to workers performing construction framing activities where:  a. On or about 8/24/2018 at 1460 Kaden Lane, Lot 78, Braselton, GA where the employer failed to ensure that step ladders were opened before use exposing workers to fall hazards of up to 6-feet to the lower level.  b.
