Jobsite:  The employer did not ensure that the employees engaged in installing corrugated metal sheathing and insulation as part of the roofing system were protected from fall hazards by the use of appropriate fall protection devices/systems.  On 09/04/2018, one of employees fell from the roof of the building while performing the above tasks and sustained two (2) broken ankles from the incident.
