29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift.   On or about September 4, 2018,  in an aerial lift basket,  an employee was exposed to fall hazards when operating a Genie aerial lift without a body belt with lanyard attached to the basket.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  On or about September 4, 2018,  on the south side of roof,  an employee was exposed to fall hazards when installing panels to a parapet wall without a fall protection system.
