29 CFR 1926.102(a)(1): 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  On or about September 18, 2018, five employees were using drills and hammers to install roofing materials and none were wearing eye protection, exposing the employees to the hazard of being struck by flying debris.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems.  On or about September 18, 2018, four employees were installing TPO roofing on top of the awning of the commercial property under construction, exposing the employees to a fall hazard over 6 feet.

29 CFR 1926.503(a)(1): The employer did not train each employee who might be exposed to fall hazards to enable them to recognize fall hazards and the procedures to be followed in order to minimize these hazards:  On or about September 18, 2018, and at times prior thereto, employees working on the commercial property under construction had not received training on the use of fall protection prior to being allowed to work in elevated places, exposing the employees to fall hazards ranging from approximately 11 feet.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about September 18, 2018, the Werner 16 Foot Extension Ladder was set up with approximately 8 inches about the awning rooftop, exposing employees to a fall hazard of approximately 11 feet.
