Old Peachtree, Peachtree Corners, Georgia:  Employees performing erosion control manufacturing activities were exposed to smoke inhalation and burn hazards when the employer did not instruct employees in the hazards of fires and portable fire extinguisher use.

Old Peachtree, Peachtree Corners, Georgia:  Airborne exposures to 80/20 Solvent Blend (Fast) which is a mixture of propyl acetate (10-30%) and Propan-2-ol (1-5%) were not evaluated for employee(s) working with the solvent and reporting headache and dizziness symptoms.

Old Peachtree, Peachtree Corners, Georgia:  Employees performing erosion control manufacturing activities were exposed to dizziness, and drowsiness, while working with solvents when wearing non-mandatory use respirators supplied by the employer when the employer did not provide employees with the information contained in Appendix D.
