29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:      The employer did not ensure that employees exposed to chemicals such as, but not limited to, welding fumes (metal fume fever) received training on those chemicals prior to their initial assignment.     Premium Contractor Solution, LLC was previously sited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.1200(h)(1), which was contained in OSHA inspection #1209122, citation 1, item 2b, and was affirmed as a final order on June 20, 2017, with respect to a workplace located at 2601 West Stroop Rd, Suite 500, Moraine, OH 45439.

29 CFR 1910.1200(h)(3)(iv):   The details of the hazard communication program developed by the employer, did not include an explanation of the labels received on shipped containers and the workplace labeling system used by their employer; the safety data sheet, including the order of information and how employee could obtain and use the appropriate hazard information:      The employer did not ensure that employees received training on the employer's hazard communication program and the location of the safety data sheets before being allowed to weld on steel (metal fume fever).     Premium Contractor Solution, LLC was previously sited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.1200(h)(1), which was contained in OSHA inspection #1209122, citation 1, item 2b, and was affirmed as a final order on June 20, 2017, with respect to a workplace located at 2601 West Stroop Rd, Suite 500, Moraine, OH 45439.
