29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:      a.) 21 Lake Mohawk Country Club Boardwalk, Sparta, NJ        Employees were engaged construction activities from a scaffold platform without utilizing any means of a personal fall arrest system, exposing them to a fall hazard approximately 30ft. to the ground below, on or about 10/02/2018.

29 CFR 1926.453(b)(2)(iv): Employee(s) working in an aerial lift were using planks, ladders, or other devices for a work position:    a.) 21 Lake Mohawk Country Club Boardwalk, Sparta, NJ       Employees were engaged in construction activities from a scaffold platform that was resting on the top rail of an aerial lift, exposing them to an approximate 30ft. fall hazard, on or about 10/02/2018.
