The Machinist was exposed to a noise level of 88.10 dBA during a 440 minutes sampling period; and,  b. The CNC Router Machine Operator was exposed to a noise level of 86.44 dBA during a 467 minutes sampling period.

An effective respiratory protection program would include, but not limited to:  a. Procedures for selecting respirators for use in the work place; b. Medical evaluation of employees required to wear respirators; c. Valid fit testing procedures for each type of tight-fitting respirators; d. Procedures and schedules for cleaning, disinfecting, storing, inspecting, repairing, discarding and maintaining respirators, and; e. Training of employees in the proper use of respirators.

Work surfaces were not regularly cleaned to prevent accumulation of lead.  Monitoring was conducted to ascertain lead presence and exposure. On October 11, 2018 the wipe samples were taken to ascertain lead exposure from different work surfaces, such as, but not limited to lunchrooms, employees work stations and desks, as well from employee shoes and hands surfaces.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:   At this establishment, employees engaged in manufacturing of different precision plastic and metal parts were exposed to hazardous chemicals that contain acids, alkali and caustics. The employer had not implemented a hazard communication program which would address labeling and other forms of warning on chemical containers, Safety Data Sheets, and employee information and training on the hazards associated with chemicals used on this site.   The written program must also contain the following:   1. A list of all hazardous chemicals on site;  2. The methods the employer will use to inform employees of the hazards associated with non-routine tasks involving chemicals, such as a spill;  3. The hazards of chemicals contained in piping that is not labeled; and,  4. The method the employer will use to inform other employers (contractors) of the chemicals their employees might be exposed to while performing duties at this site.
