Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system: On or about October 10, 2018 employees were exposed to fall hazards of approximately 16 feet. Employees were engaged in commercial roofing activity on a low slope roof with no fall protection.

The employees were using a ladder that was not extended 3 feet above the roof and no grasping device was provided to assist employees in mounting and dismounting the ladder.
