29 CFR 1926.602(a)(2)(i): Material handling equipment with rollover protective structures or adequate canopy protection was not provided with seat belts.    (a) On or about September 24, 2018, at 3798 NW 1st Terrace in Cape Coral, FL, an employee operating a front-end loader was pinned below the vehicle  when the loader came off the trailer.  The front-end loader was not equipped with a seat belt.

29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours:    a) On or about September 24, 2018 at 3798 NW 1st Terrace, Cape Coral, FL, an employee suffered a work-related injury and was hospitalized.  The employer was aware of the in-patient hospitalization on September 24, 2018 at around 12:30 P.M.  The employer failed to notify OSHA of the in-patient hospitalization.
