29 CFR  1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:  a. On  May 6, 2022, on Burnt Store Road, in Punta Gorda, Florida, employees, working in an unsupported trench that was 17 to 20 feet long and approximately 7 feet deep with vertical walls, in Class C soil, were not adequately trained in the recognition of hazards associated with trenching and excavations.

On May 6, 2022, on Burnt Store Road, in Punta Gorda, Florida, employees were exposed to cave-in hazards when working without a protective system in a trench that was 17 to 20 feet long and was approximately 7 feet deep with vertical walls, in Class C soil.
