This was most recently observed at 4822 24th St, Omaha, Nebraska on October 18, 2018 when an employee was observed working from a scaffold level that was 20' above the ground and it was not fully planked.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.    The employer is failing to protect employees from fall hazards while working from a scaffold.  This was most recently observed at 4922 24th St, Omaha, Nebraska on October 18, 2018 when an employee was observed using the cross braces and the end frame of a scaffold to ascend to the 3rd level which was 20' above the ground.
