29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of   powered industrial truck were performed by the employer without the manufacturer's prior written   approval:       On 9/17/2018, employees of Express Granite, LLC located at 1408 Harvestowne Industrial Drive, Saint Peters MO were operating a Caterpillar model GC25K forklift with Abaco AFJ-25 forklift boom attachment.  The attachment was used for moving large slabs of cut rock used in countertops.  The employer did not receive approval from Caterpillar (Forklift Manufacturer) to use the Abaco AFJ-25 Forklift boom.

29 CFR 1910.178(l)(1)(ii): Prior to permitting an employee to operate a powered industrial truck (except for training purposes), the employer did not ensure that each operator had successfully completed training.    On 9/17/2018, employees of Express Granite, LLC located at 1408 Harvestowne Industrial Drive, Saint Peters, MO were operating a Caterpillar Model GC25K powered industrial truck.  The employer had not ensured that each powered industrial truck operator was competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training.

1910.178(q)(7):  Industrial trucks were not examined before being placed in service.  Such examination was not made at least daily.     On 9/17/2018, Express Granite, LLC was operating a Caterpillar Model GC25K Forklift which had not been examined daily.  The Caterpillar Forklift had defects identified but was not removed from service.    1.)    Seatbelt was not present on the forklift.   2.)    The backup alarm was intermittent because the wires had a poor connection or short.  3.)    Plywood was zip tied to the overhead guard obscuring the drivers visibility when picking up loads.

29 CFR 1910.213(d)(1):  Each circular crosscut table saw was not guarded by a hood which shall meet all the requirements of paragraph (c)(1) of this section for hoods for circular ripsaws.     On 9/17/2018, employees of Express Granite, LLC located at 1408 Harvestowne Industrial Drive, Saint Peters, MO were exposed to an unguarded saw blade on a Delta Shopmaster Saw.  The Delta Shopmaster Saw was missing the machine guarding exposing workers to caught-in hazards.

On 9/17/2018, employees of Express Granite, LLC located at 1408 Harvestowne Industrial Drive, Saint Peters, MO were exposed to compressed air for cleaning in excess of 60 psi.  The compressed air was not chip guarded or reduced by a regulator exposing employees to struck-by hazards.

Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, the employer did not report the in-patient hospitalization, amputation, or loss of an eye to OSHA.    On 9/17/2018, while conducting a complaint inspection at Express Granite, LLC located at 1408 Harvestowne Industrial Drive, Saint Peters MO. An employee injury which occurred on 8/10/2018 was identified. Express Granite, LLC did not report the in-patient hospitalization within twenty-four (24) hours of the hospitalization.
