29 CFR 1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, including, but not limited to Items of this section:    (a)  Steam Presses:  On October 31, 2018 and times prior, the employer has not developed a specific energy control procedure for the Unipress mushroom steam presses for maintenance employees that conduct lockout/tagout on the machine during servicing and maintenance. The mushroom presses has multiple energy sources, 120 volt electrical, 80 psi pneumatic pressure, and 100 psi steam pressure.

29 CFR 1910.147(c)(6)(i):  The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed:   (a)  Facility Wide: On October 31, 2018 and times prior, employees used energy control procedures when performing service and maintenance of machinery such as, but not limited, to the Unipress mushroom steam presses (120 volt electric, 80 psi pneumatic, 100 psi steam), Multimatic dry cleaning machines (3 phase 240 volt electric), and the Honeywell red eye boiler controller (120 volt electric).  An annual periodic inspection of each energy control procedure and a review of each employee's responsibilities under the energy control procedure was not performed.

29 CFR 1910.303(g)(1): Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment:     (a)  Boiler Area:  On October 31, 2018 and times prior, the area around the 240 volt electrical disconnect and on/off switch for the air compressor was blocked by drums, a dolly, welding cart and other excess materials which did not provide sufficient access and working space.
