29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:  a) At 3650 Airport Blvd Mobile, Al (Far Southeast Front: Canopy Area): On or about October 30, 2018, the employer failed to ensure exposed employee(s) were using a body belt with lanyard attached to the approved anchor point while using an aerial lift to perform sheet metal constructions; exposing employee(s) to catapult hazards and fall hazards of approximately seventeen (17) feet to the concrete below.

29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a) At 3650 Airport Blvd Mobile, Al (Far Southeast Front: Canopy Area): On or about October 30, 2018, and at times prior; employer failed to ensure exposed employee(s) were wearing hard hats to perform sheet metal work activities at, near and beneath an elevated Genie S-65 boom aerial lift basket; exposing employee(s) to struck-by hazards from falling or flying materials and tools.
