29 CFR 1910.176(a): Where mechanical handling equipment was used, sufficient safe clearances were not allowed for aisles, at loading docks, through doorways or wherever turns or passage had to be made:    On or about October 29, 2018, at the loading dock area of 9505 NW. 108 Avenue, in Medley,  Florida, powered industrial trucks were operating and there were no designated walking areas thus exposing employees to a struck by hazard.

29 CFR 1910.178(l)(1)(ii): The employer did not ensure that each operator had successfully completed the training required by paragraph (l), except as permitted by paragraph (l)(5), prior to permitting an employee to operate a forklift:    On or about October 29, 2018, at the loading dock area of 9505 NW. 108 Avenue, in Medley, Florida, the employer failed to prevent uncertified forklift operators from operating powered industrial trucks, thus exposing employees to being struck-by the equipment.

29 CFR 1910.178(n)(6): Industrial truck drivers were not required to look in the direction of, and keep a clear view of the path of travel:    On or about October 29, 2018, at 9505 NW. 108th Ave. in Medley, FL, an employee was operating a forklift in reverse without looking in the direction of travel, thus exposing employees to a struck-by hazard.
