29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee did not affix a personal lockout or tagout device to the group lockout device before working on the machine or equipment:  a)  Production Floor; On or about November 8, 2018 and at times prior thereto the employer exposed employees to potential hazardous energies in that employees were allowed to enter and clean robotic cells including but not limited to the ADA Spot Welder Cell without all employees applying a personal lockout device on the energy control lockout point(s).

29 CFR 1910.151(b): There was neither an infirmary, clinic, or hospital used for the treatment of all injured employees in near proximity to the workplace nor a person or persons adequately trained to render first aid:      a) 10 Industrial Park, Fort Deposit AL; On or about November 8, 2018 and at times prior thereto employees were exposed to potential medical emergencies in that emergency treatment services were not within reasonable proximity of the worksite and the employer did not have a trained first aid provider at the worksite.
